Step 1:IF You r an existing user then login

step 5:Then choose date,doctor's Name..
step 6:choose shift when u want to come ,the choose your preferable Timeslot.Then Click Ok.
step 7:fill the form and submit it.After that A confirmation message will come to your given Contact number.
